Welcome to J.W. Alvey Elementary School. The School Clinic is here to support student learning. My name is, Anita Sprague RN BSN. I have over 25+ years of nursing experience in areas ranging from in-hospital, outpatient, public health, and school health nursing. Below are forms you can download if needed if your child has specific health needs that are necessary during school hours. Students are prohibited from carrying any medications without proper parental and physician written consent on file at school (updated annually), and medications are preferably to be kept in the School Clinic. Students may not bring any medication to or from school at any time.

Mandated hearing and vision screenings will be completed for all students who are new to Prince William County Schools and annually for all third grade students.
Below is information concerning Scoliosis for parents of all Fifth Grade Students. Scoliosis can occur during the growing years, especially during the growth spurt from 10 to 17 years of age. Please review the below information, and if you have any questions please feel free to call and/or contact your child’s physician if you notice any problems.

All fifth grade students are required to have a Tdap booster (tetanus, diphtheria, pertussis), immunization prior to being admitted sixth grade in Middle School. You can check your child’s school immunization status on your child’s School Fusion school account. Please review your child’s immunization status, and bring in written confirmation from your child’s doctor of immunization completion to school to have your child’s immunization status updated.
